JOB SUMMARY
Reporting to the Medical Director in collaboration with the Executive Director, the PACE Clinic Services Manager oversees and manages the clinical operations of a Program of All-Inclusive Care for the Elderly (PACE) program, ensuring the 24/7 delivery of high-quality, patient-centered care. This role involves leading the clinical nursing team, providing primary care services, developing policies and procedures, and ensuring regulatory compliance. The Clinic Services Manager is responsible for the overall quality, safety, and efficiency of clinical services, including patient assessments, diagnoses, treatments, and follow-up care.
In addition to the administrative functions the Clinic Services Manager is responsible for assessing, coordinating, monitoring, and providing health care services and treatment for Edenbridge PACE program participants.
Leadership & Staff Management
- Hire, train, mentor, and evaluate clinic personnel, including but not limited to Registered Nurses, Medical Assistants, and other healthcare team members.
- Foster a collaborative, high-performing team environment through coaching, feedback, and team-building initiatives.
- Set clear performance expectations, delegate responsibilities appropriately, and hold staff accountable to clinical and operational standards.
- Oversee scheduling to ensure adequate clinical coverage, monitor timekeeping, and manage overtime in accordance with productivity goals.
- Conduct strategic workforce planning and lead proactive recruitment efforts to support clinic needs.
Clinical Operations & Care Delivery
- Direct day-to-day clinical operations to ensure safe, efficient, and participant-centered care.
- Coordinate participant services with both internal departments and external partners such as hospitals, skilled nursing facilities, and DME vendors.
- Supervise clinic equipment inspections and ensure timely maintenance and repairs.
Care Coordination & Participant Services
- Manage participant appointment schedules, clinic workflows, and medication distribution processes to ensure smooth operations.
- Collaborate with the Interdisciplinary Team to develop and implement individualized care plans.
- Address participant concerns promptly and professionally, ensuring participant rights, dignity, and satisfaction are upheld.
- Partner with the Medical Director to create and maintain clinical protocols, workflows, and documentation standards
Provider Care Functions
- Involved in the development and implementation of QAPI activities.
- Responds promptly to questions and concerns raised by IDT members related to participant health concerns.
- Facilitate the integration of new participants into the Edenbridge PACE program health care delivery system, including medication, immunizations, and routine monitoring of chronic problems and care plan development.
- Provides regulatory primary care assessments and clinically manages chronic medical illnesses as assigned.
- Provides acute episodic evaluation and treatment of medical conditions as assigned.
- Consults with primary care providers in the event of an episodic illness when needed; assists in coordinating services provided by primary care providers.
- Monitor and order participants’ medication needs as a part of the primary care function.
- Assist participants in maintaining optimum health; provide health education and counseling to participants and caregivers facing chronic conditions and end-of-life issues.
- Participate in the on-call coverage on a rotating basis.
Quality Improvement & Compliance
- Lead the clinic's involvement in the Quality Improvement Program (QIP), tracking performance data and implementing quality enhancement initiatives.
- Conduct routine audits of Electronic Health Records (EHR) for compliance with documentation standards and accuracy.
- Ensure compliance with CMS, DHCF, OSHA, and CLIA regulations through ongoing monitoring and staff training.
- Participate in investigations of adverse events, including Level II incidents and Root Cause Analyses, and assist with policy revisions as needed.
- Organize and review emergency preparedness drills, recommending and implementing improvements.
Fiscal Management & Reporting
- Maintain fiscal responsibility for clinic operations, including oversight of expenditures, pharmacy invoices, and supply chain management.
- Monitor and manage team payroll, overtime, and clinic resource utilization to stay within budgetary constraints.
Team & Organizational Collaboration
- Collaborate with the Medical Director, Quality Department, and executive leadership to support strategic initiatives and program development.
- Promote a culture of accountability, safety, and continuous improvement across all clinic functions.
- Foster effective communication with internal departments, external partners, and stakeholders.
Training & Professional Development
- Participate in required continuing education, regulatory training, and leadership development programs.
- Provide staff with ongoing training, education, and development opportunities to maintain compliance and enhance performance.

QUALIFICATIONS
- Must be a licensed Advanced Registered Nurse Practitioner with a current DC APRN License.
- Minimum master’s degree in nursing.
- Experience as an NP in a community-based or long-term care setting is preferred.
- Experience: Prior experience as a medical staff supervisor/ clinic manager serving the geriatric population preferred.
- Management and leadership skills required.
- Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public.
- Experience crafting policies, procedures and other written processes.
- Ability to actively participate in strategic planning for both the Home Care Department and the overall PACE program.
- Ability to communicate clearly and effectively, both verbally and in written formats.
- Proficient in Excel and Microsoft Office programs, including Outlook.
- Ability to work independently with minimum supervision.
- Skilled in identifying and recommending problem resolution.
